Registration with Etowah Pediatrics is a 3 step process.
- Register online and create an account by clicking the PATIENT PORTAL button. (See screen shots of process below)
- Click the NEW PATIENT FORMS button. Print out, fill out, and bring forms with you to the office.
- Someone from the office will contact you within 2 business days to complete your registration.
Online Registration Process
Follow the steps below to complete your registration:
Click create account.
Enter your first name, last name, email, password, security code then click create account.
You will see the following message:
“Your account has been created and a welcome email has been sent to (your email). Before you can log into your account, you need to click the account verification link in the welcome email.”
When you click the link in your welcome email you will see the following message,
“Your email address has been verified and your portal account is now active. Click here to go to the main page. Please sign in using your email and password.”
Click the underlined phrase.
You will see a link to continue registering your family. Please fill out all information, including questions on far right side.
When finished registering all family members, click “Submit” to complete.
