Medical Records
If you are new to our practice it is vital that we have your child’s medical records from their previous providers prior to being seen in our office. We want to provide your child with the best care and to do so we must understand their full medical history. Therefore it is important we have your child’s medical record, including immunization data, on file prior to their first visit with us.
