Initial Evaluations
Please bring the following items with you to your initial evaluation office visit:
- All report cards for this school year. If report cards haven’t been issued, then an up to date printed list of grades will be ok. Accessing grades on the phone is not adequate. Please bring a print out of report card/grades.
- Your child’s class schedule, including time of each class.
- A COMPLETED Parent Vanderbilt Initial Evaluation Form
- A COMPLETED Teacher Vanderbilt Initial Evaluation Form

Re-Check / Follow-Up Visits
Re-Checks will be every 3 months. Some visits may be eligible to be telemedicine for your convenience.
Please bring the following items with you to your re-check visit:

- A COMPLETED Teacher Vanderbilt Follow-Up Form
*For telemedicine visits, completed forms must be turned in to the office the day before the visit.
